*Kits come with request forms which you will use to designate the hormones you wish to test/track. Request forms, often called Referral Forms, are necessary for testing of human biological tissue in Australian laboratories.
Replacement of lost or damaged test kits will be provided upon request. Maximum per purchase is 4.

Process:
- Purchase test, have it delivered to your doorstep. Tracking number available on request. Now sent via Express Post 7 days a week.
- Perform simple saliva sample collection and complete the included referral form.
- Post your sample and referral form back to the lab’ using the Express Post satchel. Tracking included.
